Stuttering is a disorder of speech fluency. The person knows exactly what they want to say, but speech is interrupted: sounds or words repeat, a sound is prolonged, or the word blocks and will not come out.

Stuttering does not come from nervousness, and it is not an expression of shyness or of low intelligence. It can be accompanied by tension, avoidance of words and fear of speaking, and that side of the experience is part of the treatment - not just the mechanics of speech.

Signs of stuttering

  • Repetition of sounds or syllables at the start of a word.
  • Prolongation of a sound.
  • Blocking: the word will not come, sometimes with visible tension.
  • Tension in the face, neck or body while speaking.
  • Substituting words the person finds difficult.
  • Avoiding situations that require speaking.
  • Fear or embarrassment attached to speaking.
  • In children: conscious awareness of the difficulty, up to falling silent.

When to seek help

At one stage of development some children pass through transient word repetition, when language is expanding faster than the ability to organise it. A proportion of these cases resolve on their own.

Assessment is called for when the difficulty persists, when tension appears, when avoidance sets in, or when the child becomes aware of it and reacts to their own difficulty. That reaction is a sign not to wait on.

In adults, stuttering rarely resolves by itself - but it is treatable at any age, and that should be said plainly.

How the assessment works

The assessment looks at the type of interruptions, how often they occur, in which situations they worsen and in which speech eases. Stuttering is rarely the same across all contexts, and that variation is valuable information.

We also assess the experience itself: how much the person suffers for it, how much they avoid, how it affects school, work or relationships. Without that side, treatment would be partial.

How therapy works

Treatment uses Professor Kaçka's holistic method (HMK), for which the practice is licensed. Holistic means the work is not limited to speech techniques but also addresses breathing, tension, rhythm and the person's attitude towards speaking.

Objectives are built step by step, and the move from one step to the next happens when the previous one is stable. The most delicate part is transfer: holding fluency outside the clinic, in real situations.

There is no guaranteed timeframe. Stuttering fluctuates even during treatment, and that is part of the process, not a failure.

The parent's role

How the family responds to stuttering carries real weight. Parents receive concrete guidance: how to speak more slowly and without hurry, how to give the child time, and why you should not interrupt, complete the sentence, or say 'calm down and try again'.

The child needs to experience their speech being received calmly. That is the biggest change a parent can make, and it starts on day one.
